I am writing to cancel my contract with you with effect from the earliest date allowed by the notice period in my contract.

Please confirm the following to me in writing: - that you have received and actioned this cancellation; - the final date on which the line or service ends; - the final bill, and how I should return any equipment on loan.

Want to keep your number? Do not cancel before your new provider has arranged the transfer, or you may lose the number.
Check whether you are still in a minimum contract term; if so, an early termination charge may apply.
Return borrowed equipment such as a router and ask for a receipt.
Keep your final bill and your proof of posting, in case of a later dispute over charges.
